Implant Types

Implants come filled with either a sterile saline solution or silicone gel and are both covered with a silicone elastomer shell.  Each type of implant has its advantages and both have been approved for their safety and efficacy by the FDA.  Implant size, shape, and fill depend heavily on the patient’s aesthetic goals, but may also depend on their anatomical structure as well.  Factors like body frame, size of chest wall, shoulder width, native breast tissue, and weight all contribute to the final decision on implant type.  Dr. Black and Dr. Arnstein take each one of these elements into consideration, along with the ultimate goal of the patient, in order to develop a plan of care that will achieve the most successful, natural results.  

 

Incision Placement

The incision required to insert the breast implant is quite small at about 3 cm, but may be a little bit longer to accommodate for silicone gel implants.  The incision can be made in several different locations:

  • Inframammary Incision-The inframammary incision is made in the fold or crease of the bottom of the breast.  This incision is very inconspicuous and can be easily camouflaged. 
  • Periareolar Incision-The periareolar incision goes along the bottom of the areola (the colored nipple area) where the color changes.  Again, this incision blends quite well and is barely visible once it heals.
  • Transaxillary Incision-The axillary incision is made in the armpit and leaves no incision on the actual breast mound.  
  • Transumbilical Incision-This is incision is made in the belly button but is not as commonly used.

 

Location Of Implant

Your implants can be placed either in the submuscular or subglandular position.  This is commonly referred to as “under or over the muscle.”  Each one has specific pros and cons which Dr. Arnstein or Dr. Black will carefully review with you.  

The subglandular position of implants refers to implants that are placed behind the breast tissue.  This may be a good option for women with adequate natural breast tissue to begin with as athletic patients who work their pectoral muscle frequently because they will not have to worry about their workout interfering with their breast implant.

A submuscular position of implants places the implants behind the pectoral muscle.  It is important to note that only the top third of the implant is actually behind the muscle.  This is ideal for leaner patients or patients that do not have adequate tissue coverage so that the implant is less palpable and can have a more natural appearance.
